Once you have made your selection and signed the form, please fax it to 09 360 1737 or mail it to The Fairy Shop, 79 Ponsonby Road, Ponsonby, Auckland.
(To avoid disappointment, please contact the Fairy Shop to check that the time you want for your party is available.)
